• Physical Examination Form
• Tuberculosis (TB) Risk Factor Screening Form
• Medical History (online form completed by student)
• Immunization Form

Students can use MySLC to enter a chosen name that is different from their legal or given first-name. The name change will be reviewed by the Office of Diversity, Equity and Inclusion and should take effect across most systems within 3-4 business days. Typically, the first-name originally entered into the Common App at the time of application to the college is the first-name entered into SLC’s systems by default. https://my.slc.edu/chosenname
